In psp open the template resize 85% all layers checked

delete these layers:

copy of raster8

Click on raster6 selection select all float defloat copy and paste a paper of choice

 selection invert delete select none delete the original

Do the same to these layers as you did in #2 above

Copy and paste your tube of choice resize if needed position in middle

Copy and paste wordart1 image resize 60% position at bottom of tag

From the kit copy, paste and resize elements of choice

 position where you like see my tag as reference

Add your name and copyright

Click on bottom white layer delete

Save as a png

ALL DONE
